Instructions:

If you do not want to participate in this settlement, you must opt out in writing by sending a request, postmarked by no later than March 27, 2009 to:

The opt out must state your name, address, telephone number, social security number and the following declaration: “I wish to be excluded from the Adema Settlement Class in the Superior Court of California, County of San Diego, Case No. GIC 866561. I understand that by requesting to be excluded from the Settlement Class, I will not receive any monies from the Settlement. I further understand that any monies I would have received will be redistributed, pro-rata, to the remaining class members and/or Defendants.”

File Objection: April 3, 2009
Fairness Hearing: April 15, 2009 at 8:30 am
